Proceed to Guidance Office for the Entrance Examination & Issuance of Result Testing Fee : P150.00

Basic Education Faculty Room
Proceed to the Basic Education Faculty Room for Reading Tests and Interview. Submit admission requirements to the Faculty-In-Charge of the Enrolment :
Kindergarten:
- PSA-issued Birth Certificate
- Baptismal Certificate
- 2 copies 2x2 ID Picture (with green background)
Elementary:
- ECCD Checklist and Kindergarten Report Card (for G1)
- SF9 Report Card (for G2-G6)
- Certificate of Good Moral Character
- PSA-issued Birth Certificate
- Baptismal Certificate
- 2 copies 2x2 ID Picture (with green background)
- Official Entrance Test Result from the Guidance Office
Junior High School:
- SF9 Report Card
- Certificate of Good Moral Character
- PSA-issued Birth Certificate
- Baptismal Certificate
- 2 copies 2x2 ID Picture (with green background)
- ESC (for ESC Grantee)
- Official Entrance Test Result from the Guidance Office
Senior High School:
- SF9 Report Card
- Certificate of Good Moral Character
- PSA-issued Birth Certificate
- Baptismal Certificate
- 2 copies 2x2 ID Picture (with green SHS QVA Certificate or ESC Certificate (for Private G10 Completer)
- Official Entrance Test Result from the Guidance Office
- 2 pieces of long brown envelopes

OTHER REQUIREMENTS:
- Foreign-born students/pupils whose parents are both Filipinos Photocopy of Valid Philippine Passport
- Foreign-born students/pupils whose parents are Non-Filipino & Filipino Photocopy of Valid Philippine Passport or Certificate of Recognition as Filipino e
- Native-born Permanent Resident Aliens (children of Non-Filipino born in the Philippines) Alien Certificate of Registration )

Library
Proceed to the Library for the list of textbooks.

Cashier’s Office
Proceed to the Cashier’s Office for Payment of fees.

Enrolment Form
Fill-up the Enrolment Form completely and legibly.

Library
Proceed to the Library and present the Official Receipt of Payment of Books.

Computer Laboratory
Proceed to the Computer Laboratory and present your I.D. stub for picture-taking.

Faculty Room
Proceed to the Faculty Room to surrender your Enrolment Form to the Faculty-In Charge of the enrolment.
New Student - College

- SF9 Report Card (Senior High School Report Card) (Original Copy)
- Certificate of Good Moral Character (Original Copy)
- One (1) photocopy of PSA Certificate of Live Birth
- One (1) Photocopy of Baptismal Certificate
- Two (2) Long Brown Envelope
- One (1) Long Plastic Envelope
- Two (2) pieces: 2x2 I.D. pictures
- One (1) photocopy of Marriage Contract (if married women)
Cashier’s Office
Guidance Office
(Guidance staff will issue medical laboratory referral slip to BSN,BSM & BSRT students)
Dean’s Office - Interview
- Nursing & Midwifery Faculty Room – for BSN & BSM Students (interview, then submit medical lab. requirements)
- BSRT Faculty Room – for BSRT Students (interview, then submit medical lab. requirements)
- Gen. Ed. Faculty Room – for AB,BSED,BEED,BSBA,DST students (interview only)
- Graduate Studies Dean’s Office – for MAED Students
